Position Overview
The Care Plan Project Manager at the Bridge Housing Campus is responsible for the administrative coordination and data management of resident care plans, as well as the organization of resident schedules and medical appointments. This position collaborates closely with the Associate Director of Housing and Advocacy to ensure that each resident’s care plan aligns with the goal of transitioning to stable, independent housing. The Care Plan Project Manager supervises the Care Plan Coordinator who together provides essential support to the Caseworkers, Residential Community and Culture Manager, Associate Director of Housing and Advocacy, and other team members in helping to streamline care plan processes and foster residents’ progress toward housing stability.
Key Responsibilities
1. Care Plan Coordination and Data Management
- Care Plan Integration: Collaborate with Caseworkers to standardize and streamline individualized care plans, ensuring consistency in approach and alignment with resident needs.
- Documentation and Records: Maintain accurate records for care plans, resident interactions, and progress notes. Ensure data is documented efficiently, accessible to relevant staff, and up-to-date.
- Data Tracking and Reporting: Monitor and report on resident data related to care plans and resident outcomes, preparing information for program evaluations, grant reporting, and internal reviews.
2. Resident Scheduling and Appointment Coordination
- Medical Appointment Coordination: Schedule and coordinate resident medical appointments, both on-site (e.g., with CHASS) and off-site, ensuring timely access to necessary care and effective communication with healthcare partners.
- Daily Scheduling Support: Work with Caseworkers to coordinate and oversee resident schedules, including programming, care plan reviews, one-on-one sessions, and additional appointments.
- Appointment Reminders and Follow-Up: Provide appointment reminders and follow-up coordination, ensuring residents have necessary information and are prepared for scheduled services.
3. Administrative Support for Resident Services
- Communication with Team: Act as a central contact for Caseworkers and the Residential Community and Culture Manager regarding care plan updates and resident scheduling. Ensure all relevant staff are informed about resident schedules, appointments, and support needs.
- Resource Allocation: Assist the Associate Director in coordinating resources to meet residents’ care plan requirements, including medical and personal support needs.
- Policy and Procedure Support: Support the development and refinement of care plan and scheduling policies, ensuring best practices in resident care coordination and program alignment.
4. Project Coordination and Continuous Improvement
- Alignment with Housing Goals: Work closely with the Associate Director of Housing and Advocacy to ensure each resident’s care plan includes steps and resources for achieving housing stability.
- Coordination with Community Partners: Maintain relationships with external agencies, such as CHASS and local hospitals, to facilitate seamless resident medical support and access to external resources.
- Feedback Collection: Gather feedback from Caseworkers and program staff on care plan and scheduling processes, providing insights to the Associate Director for continuous improvement.
Qualifications
- Education: Bachelor’s degree in Social Work, Psychology, Human Services, or a related field, or equivalent experience in administrative support for social services or case management.
- Experience: Prior experience in project coordination, data management, or administrative support in a social services, healthcare, or residential setting.
- Skills: Strong organizational and data management skills; proficiency in documentation and record-keeping. Familiarity with case management software is beneficial.
The Care Plan Project Manager will play a crucial role in the success of the Pope Francis Center Bridge Housing Campus, ensuring that residents receive the comprehensive support they need to transition to stable, independent living.
Working Conditions:
- This job primarily operates in-person in a dynamic social services setting. This role requires the ability to lift 50 pounds independently; walk, climb, or descend stairs; kneel, bend, and twist; and occasionally operate a commercial vehicle to pick up and deliver items.
